For Blood Draws – Book under the Full Circle Phlebotomist all blood draws are cash pay only – $55
New Patient Checklist
We are in-network with: Premera, Regence, BCBS, Lifewise, Anthem, Kaiser PPO, First Choice
– Patients with out of network plans will need to be self pay but can be provided with a superbill to self submit to your insurance for reimbursement- – Cash rate is $425/hr
– We are not able to bill Medicare or Medicaid plans
New patients must schedule a New Patient Appointment before booking any other appointment types.
We ask all new patients also book a 45-minute follow-up about 3 weeks after their first visit to ensure we can follow up in a timely manner.
Your appointment is not considered booked until you receive an appointment confirmation AND you have completed all the questionnaires in the CHARM patient portal, including saving a card on file.
